Don't forget to complete our patient information forms

On the day of your visit, you must bring three completed forms, Patient Information, Medical History and Privacy Notice, to the location where your treatment will be delivered. Please make sure you complete all the fields and write clearly as your information is important to us. It is also important that you bring along:

• any medications that you are currently taking
• photo ID
• insurance card (coverage must be confirmed by the patient, contact your provider to learn about your coverage)
